Step 1:
Medication errors are a significant problem in healthcare, affecting patient safety and outcomes. These errors can occur during the prescribing, transcribing, dispensing, administering, or monitoring stages of medication use. The issue is complex and multifactorial, involving individual, system, and organizational factors. As a nurse with seven years of experience, I have witnessed the impact of medication errors on patients and their families, as well as the healthcare professionals involved.

Step 2:
To conduct my search for scholarly articles, I used various databases, including PubMed, CINAHL, and Cochrane Library. After conducting a thorough search, I selected four peer-reviewed articles published within the last three to five years. The articles were chosen based on their relevance to the topic, their contribution to the current literature, and the quality of the research.

Hammoudi, Ismaile, S., & Abu Yahya, O. (2018). Factors associated with medication administration errors and why nurses fail to report them. Scandinavian Journal of Caring Sciences., 32(3), 1038–1046.
This study aimed to identify the factors associated with medication administration errors and explore the reasons for nurses’ failure to report them. The findings revealed that the most common factors contributing to medication errors were related to nurses’ workload, experience, and training. Moreover, nurses often failed to report errors due to fear of disciplinary action, lack of time, or ignorance of reporting procedures.

This study aimed to investigate the contribution of staffing to medication administration errors using text mining analysis of incident report data. The results showed that understaffing was a significant contributor to medication errors. Nurses reported that they were frequently interrupted, lacked time to perform medication administration safely, and did not receive adequate support from colleagues.

This study aimed to explore the nursing behaviors that contribute to the safe administration of medication beyond the five rights. The findings showed that nursing behaviors such as effective communication, attention to patient safety, and teamwork are crucial in preventing medication errors. The authors also suggested that incorporating these behaviors into nursing education could improve medication safety.

Karttunen, Sneck, S., Jokelainen, J., & Elo, S. (2020). Factors causing medication errors in an electronic reporting system. Scandinavian Journal of Caring Sciences., 34(1), 108–117.
This study aimed to identify the factors causing medication errors in an electronic reporting system. The results showed that the most common causes of medication errors were related to the prescription process, such as incorrect dosing and frequency. The authors suggested that developing a standardized approach to prescribing medication could reduce the occurrence of errors.

Step 3:
Developing an annotated bibliography has provided me with valuable insights into the issue of medication errors. The main points I learned from my research include the multifactorial nature of medication errors, the importance of nursing behaviors and staffing levels in preventing errors, and the need for standardized approaches to medication administration. The authors found that inadequate staffing, high workload, lack of training, and poor communication were significant contributing factors to medication errors.

The second article, “The Contribution of Staffing to Medication Administration Errors: A Text Mining Analysis of Incident Report Data” by Härkänen et al. (2020), investigates the relationship between staffing levels and medication administration errors. The authors used text mining analysis of incident report data to identify staffing-related factors that contribute to medication errors. They found that staffing levels significantly affect the incidence of medication errors.

The third article, “The safe administration of medication: Nursing behaviours beyond the five-rights” by Martyn et al. (2019), examines nursing behaviors that are essential for safe medication administration, beyond the traditional five rights of medication administration. The authors found that nurses must possess the knowledge, skills, and attitudes to ensure safe medication administration.

The fourth article, “Factors causing medication errors in an electronic reporting system” by Karttunen et al. (2020), explores the factors that contribute to medication errors in an electronic reporting system. The authors found that inadequate knowledge, poor communication, and lack of attention to detail were significant contributors to medication errors.

Through the development of this annotated bibliography, I have learned that medication errors are a significant problem in healthcare that can be caused by a variety of factors, including inadequate staffing, high workload, poor communication, lack of training, inadequate knowledge, and poor attention to detail. I have also learned that safe medication administration requires nursing behaviors beyond the traditional five rights of medication administration. These articles have enhanced my knowledge about medication errors and provided insights into how healthcare professionals can work to prevent medication errors and improve patient safety.
