case study 2––Patient is a 26 year old AA female, here for an evaluation, patient reported she has issues socializing with friends, have a lot of anger issues, , having feelings of becoming violent when some one makes her angry which she state she has been experiencing for the past three months , fluctuating moods , poor energy , racing thoughts, Pt.endorsed history of rapid mood lability, decreased need for sleep, increased goal directed activity, racing thoughts, mood swings, poor concentration and increased irritability., patient report she easily gets annoyed , she is not in good terms with her family.Patient denies AVH, denies SI/HI at this time.

Camila 0.35 mg tablet 1 tablet po daily

Past psychiatric History 

Patient report being in talk therapy following the death of his father in high school and subsequently for depression.

Patient  report history of sexual abuse from her father at age 15 and 16  and she states she has not dealt with it.  

No previous hospitalization  for mental health issues

No previous psychotropic trials

Week 4 Journal Entry
Select a client that you observed or counseled this week. Then, address the following in your Practicum Journal:

• Describe the client (without violating HIPAA regulations) and identify any pertinent history or medical information, including prescribed medications.
• Using the DSM-5, explain and justify your diagnosis for this client.
• Explain whether cognitive behavioral therapy would be effective with this client. Include expected outcomes based on this therapeutic approach. Support your approach with evidence-based literature.
• Explain any legal and/or ethical implications related to counseling this client.
