Now that you have looked at some Informed Consent examples online and in your peers’ postings in the Discussion, compose your own one page Consent for Testing Form that you would yourself be willing to sign.

Make it look like a form you would see in a professional office. You need a centered heading at the top so give it a title.

Next, list the purpose of the Consent Form and what it is being used for. In this case, testing and assessment to aid in formulation of a treatment plan.

Include risks and benefits of tests and assessments in a short paragraph or a short list.

Explain what will be done with the results of the testing and assessment: how they will be stored/kept, shared with other practitioners, and utilized in the client’s treatment. Include what the client can expect in terms of being told the results, and having a chance to review them with you as the practitioner and ask questions.

Leave a line at the bottom for the client to sign as well as print his/her/their name, and include a space for the date the signature is signed.

Your consent MUST be one page only. Do not include any unnecessary test and use Times New Roman font, point 12 or 14 size.
