In recent years, unintentional deaths in health care settings, particularly those caused by medical errors, have been on the rise. Such errors must be avoided in order to reduce the number of unexpected fatalities. It is vital to emphasize the importance of federal programs and patient safety in decreasing medical errors in health-care facilities. Patient safety is a distinct health-care area that focuses on the safety of patients while they are getting medical treatment. Furthermore, patient safety is based on the prevention, mitigation, reporting, and analysis of various medical errors that may have negative repercussions, such as patient mortality (Liu et al., 2018). Patient safety is currently considered an endemic problem by the World Health Organization. As a result, patient safety is vital in protecting patients’ interests by preventing a variety of reasons that could have negative repercussions (Khan et al., 2018). Patient safety is a field that focuses on not only patient safety in a health-care setting, but also activities that can be performed to assure patient safety with minimal effort on the part of health-care staff. As a result, patient safety is vital in promoting patients’ interests by preventing a number of situations that could have a detrimental impact on them (Khan et al.,2018). Information such as patient transfers to other health-care facilities, for example, could lead to medical errors. A lack of communication of patient findings and a detrimental impact on pharmacological prescriptions are the most common medical errors caused by a lack of information flow. In such scenarios, medical errors have always occurred, resulting in catastrophic health consequences for the patients. Medical errors in outpatient health care settings may be caused by human personnel concerns. More specifically, there have been multiple instances where people’s negligence has resulted in health care practitioners failing to ensure that suitable health care protocols, regulations, procedures, and standards are followed in a health care environment. In a health-care setting, poor labeling and record keeping can also represent human difficulties (Royce et al., 2019). The repercussions of health-care employees making mistakes due to a lack of experience are frequently devastating. The value of discussing with peers, applying expertise appropriately, and properly formulating a health care plan must be taught to health care workers. A health care provider’s responsibility is to assess the most obvious disease diagnosis and practice health care in such an automated manner that medical errors are avoided in health care settings, particularly in outpatient settings.
