Case management and care management are vital areas in the delivery of care to the public. Care management focuses on improving the quality of care to help patients with diverse diseases to cope with the conditions (Hudon et al., 2018). Case management also focuses on collaboration meant to meet the unique health needs of families and individuals. Case management, as well as care management, are essential in the delivery of care to the public through different strategies.
Case Management and Care Management
Case management is the consolidated structure of providing care, enhancing safety, managing, assessing, and promoting diverse possibilities to realize patient outcomes (Hudon et al., 2018). On the other hand, care management is a set of activities meant to improve quality of care, reduce health worker involvement, eliminate duplication, and coordination of care (Hudon et al., 2018). The purpose of care management is to help patients and clinicians to manage health conditions effectively.

In care management, the client determines the scope of work while in care management, the agency determines the scope of work. The two approaches, however, are utilized to ensure a patient’s needs are met effectively and sufficiently.

Case management utilizes various concepts to provide clinical practice in the community. The different methods are utilized as strategies to realize improved patient outcomes. The principles include justice, autonomy, beneficence, veracity, equity, and non-maleficence (Dieterich et al., 2017). For instance, the autonomy concept indicates that patients have the freedom to make decisions independently. Justice and equity also streamline the provision of care to all needy people without bias. The case manager also should demonstrate kindness and mercy while providing services. The services should not harm anyone in the community but should strive to improve their health condition.
Types of Families
A family represents people with common ancestry characteristics or relationships. For instance, a relationship between parents and children or spouses defines a family set up. The family also includes grandparents, cousins, uncles, and aunties (Thyrian et al., 2017). There are various groups of families depending on the relationship including parent and child, spouses or relatives. One of the models of care for families is the family-centered approach (Thyrian et al., 2017). The approach focuses on the medical as well as psychological needs of patients plus the impact of the family on the health condition.
Family Level to Aggregate Level
The strategy of moving from the family level to the next level is understanding that dynamics change from one level to another. The levels present unique needs that should be addressed differently (Copelli et al., 2017). The strategies include adjusting the approaches to delivering care to patients. Healthcare professionals should carry out an in-depth assessment to determine the health needs of an aggregate level. Other methods include enhancing the coverage and provision of resources. The increase in resources tackles the diverse needs of the population.
Conclusion
Healthcare services provide care to patients using different approaches, such as care and case management. The two approaches provide custom care to different categories of patients. The purpose of the diverse approaches is to reach out to different patients with unique needs. The unique needs require innovative approaches that are evolving to promote the safety and efficiency of comprehensive care. Healthcare workers utilize case and care management to improve the health conditions of families and individuals.
